Welcome to ProActive Acupuncture! We look forward to providing you with the highest quality healthcare.

Please fill out and sign all forms prior to your first visit. Our forms can be printed directly from our website or you may arrive 15-20 min early to fill them out in our office. You may also fax or email us your forms prior to your appointment.

For patients utilizing their medical insurance, make sure to have your coverage verified prior to treatment. You may call us directly or fill out the insurance verification form through the link below..

Your first visit will be approximately 60 minutes and will include an initial consultation, initial exam, and acupuncture treatment.

We recommend wearing comfortable loose clothing for your treatment; additionally we provide towels and patient gowns for patients coming directly from work.

If you need to change or cancel your appointment, please provide at least 24 hours notice.
