Please fill out and sign all downloadable forms prior to your first visit. You may fax all forms to (916) 444-3394. If you do not have access to a computer, you may fill out your intake forms prior to your first visit, however, be sure to arrive 20 minutes prior to your appointment. Intake forms are located on top of the reception desk.
Your first appointment will be approximately 60-90 minutes. Be sure to plan your schedule accordingly.
Free two hour parking is available on both Capitol Ave and 24th street.
If you have health insurance that covers acupuncture, be sure to contact the office prior to your first appointment. A copy of your insurance card and driver’s license is required to verify your coverage.
Payments, in the form of cash, check, Visa or Master Card are expected at the time services are rendered.
If you need to change your appointment, please provide at least 24 hours notice.
